Your Role as a Hospice RN:
• Provide expert, compassionate nursing care in homes and community
settings.
• Create and implement personalized care plans for patients and families.
• Offer pain and symptom management to ensure maximum comfort.
• Act as a case manager, coordinating care with the interdisciplinary team.
• Admit hospice appropriate patients to the services of Crown Hospice
• Educate, support, and empower patients and families throughout their

What You Need to Succeed:
• Graduate of an accredited nursing school with a current RN license.
• 1–2 years of acute care or home care experience (preferred, but not
required).
• Strong assessment, communication, and critical thinking skills.
• Ability to lift, position, and transfer patients as needed.
• A valid driver’s license and insured vehicle for home visits.
